Health care providers diagnosing a fetal life-threatening condition must provide pregnant women with these materials and a certification form acknowledging receipt of the information. Providers must obtain a signed certification form from the patient and retain it in their medical records. The bill imposes disciplinary action and administrative penalties on health care providers who fail to comply, including a written warning for the first violation and a $1,000 penalty for each subsequent offense.
