SB 684 requires the adoption of a uniform coordination of benefits (COB) questionnaire for all health benefit plans in Texas. The Texas Department of Insurance will develop the standardized questionnaire in collaboration with stakeholders, and all health benefit plan issuers must use it by February 1, 2026. The questionnaire will streamline the process for coordinating benefits among insurers and health care providers. The bill applies to all health benefit plans with COB provisions and includes individual, group, and public health plans, as well as Medicaid and CHIP programs. The act takes effect on September 1, 2025.
